In 30 patients with genotype-confirmed hypertrophic cardiomyopathy, global PCCT-derived extracellular volume (ECV) closely matched cardiac MRI: 30.0 ± 4.8% vs 30.62 ± 4.2% (p = 0.59), with a small Bland–Altman bias of 0.6% and limits of agreement of −1.49% to 2.75%.
Agreement was also strong regionally, with segmental correlations of r = 0.89 at the base and r = 0.85 at both mid-ventricular and apical levels. Interobserver reproducibility was excellent for PCCT-derived ECV (ICC 0.982) and MRI-derived ECV (ICC 0.921).
A global PCCT ECV threshold of 33.4% identified patients with LGE ≥15% with 80% sensitivity and 76% specificity (AUC 0.77). PCCT ECV also correlated with functional and biomarker severity, including peak VO₂ (r = −0.76) and NT-proBNP (r = 0.59), supporting its potential as a combined coronary and myocardial tissue-characterization biomarker.
