Patient:

• 80-year-old women
• hypertension, insulin resistance, dyslipidaemia
• Prior AMI (1997)
• Prior coronary intervention: small-calibre IM stent
• Current status:
  • no chest pain, physically active
  • intermittent arrhythmia episodes

Clinical context:

• pre-operative assessment

Findings:

• CaSc 980, patent stent, midRCA 50% stenosis, complete bridge, occluded pLAD
• Subendocardial fatty degeneration
• Mycardial thinning
• Apical left ventricular aneurysm without thrombus (max. diam: 22 mm)
• The sinotubular junction is circumferentially calcified
